40Summary:
The Social Worker/Service Coordinator is responsible for the development, coordination, and implementation of the Fostering Health Program protocol and intake process. This Social Worker will work closely with foster children admitted to the hospital for medical treatment providing health navigation and trauma-informed care to patients and families. The coordinator will work to build strong community relationships through collaborative partnerships with foster care agencies, working with diverse populations, and prioritizing treatment for high-risk children and families. The coordinator serves as a resource to the patients, families, community, leadership, and system partners to identify target audiences to provide program goals and accessibility. Requires occasional travel in the ten-county Region 3B service area (Tarrant, Johnson, Hood, Parker, Wise, Denton, Palo Pinto, Erath, Cooke).
Education:
Requires a Master of Social Work degree.
Two (2) years of experience working with vulnerable populations and people with chronic or complex conditions.
Bilingual English and Spanish a plus.
Required skills: relationship-building, effective written and verbal communication, critical thinking and problem-solving, ability to plan and organize effectively, proficient in use of computer applications.
Certification/Licensure:
Current Texas License for LMSW.
Basic Life Support Certification (BLS) for Health Care Providers within 30 days of employment.
